Please note: We no longer accept cheques. This is in line with other major agencies and recognises the significant decline in cheques received over the last 5 years.

The statutory declaration is a legal document, and must be witnessed in accordance with the Oaths and Declarations Act 1957.

If you're completing the form outside of New Zealand, the form must be witnessed according to your local requirements for a formal declaration.

Avoiding common errors

Follow these tips to avoid some of the common errors when calculating and paying levy: 

  • Calculate the levy payment on the amount insured or indemnity value, NOT on the premium.
  • Include GST with payment, and use the most recent GST rate.
  • Sum insured and indemnity values shown in NZD. Calculate the levy payment in NZD. When dealing with overseas policies, make sure you convert all values in the local currency to NZD.
  • Ensure bank fees/costs are paid in full by the levy payer when paying from offshore.
